You will receive an appointment reminder text to your mobile phone at least 4 days prior to your scheduled visit.
After confirming your reminder text, you will be prompted to complete all required Sadio Portal forms and consents prior to your appointment.
Consent forms must be signed on a computer or tablet (not a mobile phone)
If you cannot complete forms prior to your visit, please arrive 30 minutes before your scheduled appointment.
Medical Records
A Medical Records Release is required to obtain records from Grima Dermatology & Surgery. Please complete this within your Sadio Portal forms and consents prior to your visit.
If you would like to send medical records to Grima Dermatology & Surgery, please contact the office or physician where your care originated and request that records be faxed to (808)909-3894.
Appointment Policies - Arrivals / Late
Arrivals: Please plan to arrive at least 15 minutes early to allow adequate time for check-in.
Late Appointment Policy: Patients arriving more than 15 minutes late may be asked to reschedule. We appreciate your understanding and cooperation.
